Community Support Program

This program serves persons with severe and persistent mental illness, who have long-term needs for assistance to live in the community.  Severe and persistent usually means schizophrenia, or another disorder that puts them at risk of hospitalization in a psychiatric unit. 
 
In this program, social workers and nurses go out into the community to meet with the individual, and often go places together to take care of necessary business.  Persons in this program also see our psychiatrist for their medications that are usually necessary in the treatment of major mental illness.  Our program helps stabilize the lives of these persons, who otherwise would be very vulnerable to severe disruption due to their mental illness. 
 
Northland provides this program through contract with Marquette County.  Residents of other counties who have this need would have to talk to the office in their own county.  Our program is state certified so that we are able to bill our services to the Wisconsin Medical Assistance Program.  If you or someone you know may qualify for this service, contact our main office in Westfield.
